ABOUT CFS:
Coordinated Family Support (CFS) is a service designed for adults with developmental disabilities who choose to live with their families. CFS was created in response to input from families about the lack of available services for adult consumers who choose to live in the family home. Our staff is trained in meeting the family's goals and objectives and optimizing independence through community involvement and in-home structure as needed. Once the match is approved by the parent, Individual, or caretaker, the individual may be able to optimize their independent functioning in their home or community. Coordinated Family Supports strives to bring a "tranquil" approach to train and support both individual and family through our program. With the right match and providing one-on-one Coordinated Family Supports, the service is more effective and alleviates anxiety, frustration, and stress from parent/provider, and most importantly the individual. Our Coordinated Family Supports instructors act as mentors and positive role models for our individuals, while providing necessary training and support to keep them focused on positive life experiences. In addition, supports provided through CFS will be provided in a manner that is respectful of the culture, ethnicity, and linguistic preferences of the individual and their family.
Our Philosophy

Provide individuals and their family support necessary to successfully reside in the family home

Provide individuals and their family assistance and training for the person served and their family in navigating comprehensive services and supports that are tailored to meet their unique needs, including creating pathways to overcoming barriers to accessing generic and other resources

Provide individuals and their family additional information or resources on the diagnosis and identified supports of the person served

Coordinate consistency in training across providers specific to the needs of the person served and their family

Assist individuals and their family with scheduling of service delivery including medical and other appointments

Identify transportation options or services for the individuals and their family

Identify backup providers or supports to be available for the individual and their family

Provide futures planning for the person served living with aging caregivers or family members

Provide training to the person served and their family or circle of support which maximizes their independence

Referral Intake Process:
With the Person-Centered approach, the screening of a potential individual and assessment procedures will be a collaborative among the Regional Center, family members and our supervisor, also known as the ICS Team. After receiving a referral packet from the Regional Center Service Coordinator, our ICS Team will review a referral packet. The referral packet will consist of recent Individual Program Plan, Annual Review, Psychological report, Face Sheet, CDER outlining Individual's skills and medical reports. Utilizing the entrance criteria, the ICS shall review the packet to determine whether ICS is able to meet Individual's need. The estimated intake time, from the time that the referral packet is submitted for review to either accept or deny CFS services is five business days, If the individual seems appropriate, the ICS Team will contact the Regional Center Service Coordinator and request a POS authorization to complete an initial assessment. At a minimum, the need of the person served shall be assessed annually with progress being reported quarterly. The Regional Center should authorize the assessment up to 10 hours.
Entrance Criteria:
Ambulatory individuals, ages 18 and over who have been diagnosed as being developmentally disable. Applicants who require support, guidance and assistance while residing in their family home. The individual's IPP includes at least one objective than can be met by InClusion Services.
